Locating Day/residential services for 24-hour clients

As a Case Manager at Helping Hands Community Services LLC, one of my core responsibilities is helping individuals with high support needs access safe, reliable, and person-centered 24-hour residential care. I work closely with families, Managed Care Organizations (MCOs), and licensed providers to locate appropriate residential settings that meet each client's unique physical, emotional, and behavioral needs. I also assist in identifying and securing Day Services that promote social engagement, skill-building, and community integration. This includes coordinating tours, reviewing service models, verifying provider credentials, and ensuring that placements align with the individual’s long-term goals and preferences. My goal is to ensure every client has a stable living environment and meaningful daily opportunities that support their independence, dignity, and quality of life.
